Job Description
Responsibilities
- Develop referral relationships with: Hospitals Skilled nursing facilities Rehabilitation centers Physician offices Case managers and social workers Support coordinators and community service boards
- Conduct regular in-person outreach visits throughout Northern Virginia
- Present and promote Shalom Homecare services to healthcare professionals
- Identify and secure new patient referrals
- Maintain ongoing communication with referral sources
- Track outreach activities and referral results
- Represent the agency at healthcare networking events and community meetings
- Assist in expanding awareness of our personal care, home health, and waiver programs
Requirements
