Job Summary

Own the community partnership pipeline and field outreach to hospitals and NORCs, turning introductions into referrals and strategic alliances.

  • Partnership Development: Build and maintain relationships with community organizations, hospitals, and NORCs to fuel referral sources.
  • Outreach & Enablement: Lead in-person visits, calls, emails, and educational talks while tracking activities in the CRM.

Job Description

Responsibilities
  • Develop and maintain relationships with community organizations, hospitals, and NORCs.
  • Conduct outreach through in-person visits, phone calls, emails, and educational presentations.
  • Identify and cultivate new referral sources and partnership opportunities.
  • Maintain accurate records of outreach activities and contacts in Conversion Copilot CRM.
  • Collaborate with leadership to leverage new and existing partnerships and share market insights.
  • Attend regular team meetings and provide progress updates.
Requirements
  • Strong communication and relationship-building skills.
  • Comfortable networking with healthcare professionals and community partners.
  • Organized, self-motivated, and able to work independently.
Preferred
  • Experience in healthcare, senior services, home care, sales, marketing, or community outreach is preferred.
