Job Summary

Own the community-facing referral network for an outpatient SUD program, turning relationships into steady partner flows and smoother admissions.

  • Referral Development: Build and diversify sources with healthcare systems, hospitals, and community organizations.
  • Education & Enablement: Train partners on programs, admissions, and eligibility to speed referrals and conversions.
  • Partner Management: Maintain ongoing CRM-driven follow-up, reporting, and collaboration with admissions and clinical teams.

Job Description

Responsibilities

  • Represent Golden Health LLC professionally throughout the community.
  • Build and maintain strong referral relationships with healthcare providers, hospitals, detox centers, emergency departments, crisis teams, mental health providers, county agencies, and correctional systems.
  • Conduct regular in-person visits to referral partners throughout the Twin Cities and surrounding areas.
  • Develop new referral sources while maintaining existing partnerships.
  • Present Golden Health’s services to community organizations and healthcare professionals.
  • Attend networking events, resource fairs, coalition meetings, conferences, and community outreach events.
  • Educate referral partners on Golden Health’s treatment programs, admission process, and eligibility requirements.
  • Maintain ongoing communication with referral sources and provide timely follow-up.
  • Collaborate closely with admissions and clinical staff to ensure smooth client transitions into treatment.
  • Track outreach activities, referral sources, and partnership development using CRM or reporting tools.
