Job Summary

Own the community outreach and referral pipeline for a Northern Virginia non medical home care agency, turning inquiries from seniors and families into in home support referrals.

  • Growth/Outreach: Build and nurture relationships with hospitals, physicians, and senior care partners to source referrals.
  • Channel ownership: Represent the agency at health fairs and community events and educate families about care options.
  • Process and pipeline: Schedule in home consultations, follow up with leads, and maintain compassionate, consultative notes and handoffs.

Job Description

Responsibilities

  • Build and maintain professional relationships with: Hospital case managers, Social workers, Discharge planners, Rehabilitation facilities, Senior living communities, Assisted living facilities, Independent senior communities, Physicians' offices, Geriatric care managers, Community organizations
  • Schedule and conduct in-person visits with referral sources.
  • Present company services in a professional and knowledgeable manner.
  • Represent the company at health fairs, senior expos, networking events, and community events.
  • Distribute marketing materials and educate the community about our services.
  • Develop long-term referral partnerships.
  • Make outbound phone calls to prospective clients who inquire about our services.
  • Follow up with leads in a professional and compassionate manner.
  • Answer basic questions regarding our home care services.
  • Schedule free in-home consultations.

Requirements

  • Excellent verbal and written English communication skills.

  • Comfortable speaking with seniors and their families.
  • Strong relationship-building skills.
  • Ability to network with healthcare professionals.
  • Professional appearance and demeanor.
  • Strong organizational and time management skills.
  • Self-motivated and able to work independently.
  • Reliable transportation and willingness to travel throughout Northern Virginia.
  • Valid driver's license and proof of automobile insurance.
