Job Summary

Own community-based referral growth for home health and hospice by building and sustaining physician, hospital, and social-work partnerships, while coordinating with intake and clinical teams to speed referrals and admissions.

  • Referral Partnerships: Own relationships with physicians, hospitals, and community groups to drive steady referrals.
  • Process Ownership: Educate partners on eligibility and Medicare rules and ensure timely referral processing with intake.

Job Description

Responsibilities

  • Develop and maintain professional relationships with referral sources including physicians, hospitals, nursing homes, rehabilitation facilities, assisted living facilities, social workers, discharge planners, and community organizations.
  • Promote Home Health and Hospice services to healthcare providers and community partners.
  • Generate and increase patient referrals through ongoing relationship-building activities.
  • Conduct face-to-face visits, presentations, and educational programs for referral sources.
  • Identify and develop new referral opportunities within assigned territories.
  • Attend community events, health fairs, networking meetings, and professional association events.
  • Represent the agency in a professional and positive manner at all times.
  • Educate referral sources regarding agency services, admission criteria, Medicare guidelines, and patient eligibility requirements.
  • Assist referral sources and patients through the referral and admission process.
  • Collaborate with intake staff and clinical teams to ensure timely processing of referrals.

Requirements

  • Associate Degree or Bachelor's Degree in Marketing, Business Administration, Healthcare Administration, Public Relations, Communications, Social Work, Nursing, or a related field (Required)
  • Minimum one (1) year of sales, marketing, business development, healthcare outreach, or customer relationship experience (Required)
  • Valid Texas Driver's License (Required)
  • Reliable transportation with current automobile insurance (Required)
  • Ability to travel throughout assigned territory (Required)
  • Excellent interpersonal and relationship-building skills (Required)
  • Strong verbal and written communication abilities (Required)
  • Professional presentation and public speaking skills (Required)
  • Ability to work independently and meet referral goals (Required)
  • Strong organizational and time-management skills (Required)

Preferred

  • Bachelor's Degree in Healthcare Administration, Marketing, Business Administration, or a healthcare-related field (Preferred)
  • Two (2) years or more of Home Health, Hospice, Healthcare Marketing, Physician Relations, or Medical Sales experience (Preferred)
  • Established relationships within the healthcare community (Preferred)
  • Experience working with hospitals, skilled nursing facilities, assisted living communities, physicians, and case managers (Preferred)
  • Knowledge of Medicare and Home Health/Hospice services (Preferred)
  • Proficient in Microsoft Office and CRM systems (Preferred)
  • Knowledge of healthcare industry regulations and compliance standards (Preferred)
  • Self-motivated with a strong customer-service focus (Preferred)
