Job Summary
Own the on-the-ground relationship engine for community health, balancing patient navigation with partner growth in Virginia Peninsula.
- Build and sustain partnerships with hospitals, skilled nursing, assisted living, and community orgs to generate referrals and expand At Home Harmony programs.
- Navigate social determinants of health by connecting patients to primary care, dementia care, caregiver support, and community services.
Job Description
- Conduct in-person outreach to patients, caregivers, and community members.
- Assist individuals in navigating healthcare and community resources.
- Identify social determinants of health impacting patient well-being.
- Provide education regarding: Primary care services
- Dementia support services
- Chronic disease management
- Caregiver resources
- Community programs and benefits
- Connect individuals with available healthcare, social service, and community resources.
- Support care coordination efforts between patients, caregivers, facilities, and healthcare providers.
- High school diploma or equivalent.
- Strong interpersonal and communication skills.
- Ability to establish and maintain professional relationships.
- Reliable transportation and ability to travel throughout assigned territory.
- Proficiency with Microsoft Office and electronic documentation systems.
- Ability to work independently while maintaining accountability.
- Community Health Worker Certification.
- Experience in healthcare, senior services, community outreach, sales, marketing, social services, or patient advocacy.
- Knowledge of healthcare systems and senior care resources.
- Existing relationships within the Virginia Peninsula healthcare community.
