Job Summary

Own the community engagement program to grow and retain members by planning events, nurturing partnerships with senior-focused organizations, and turning grassroots activity into joined membership and ongoing engagement.

  • Lead multi-channel events (health fairs, doc talks, sponsorships, social events) to raise visibility within diverse senior communities.
  • Track grassroots leads and ensure timely follow-up to convert interest into membership.
  • Collaborate on outreach and marketing strategies tailored to senior populations and managed communications (Campaign Monitor email blasts).

Job Description

Responsibilities

  • Build and sustain strong relationships with community-based organizations to promote membership growth and retention.
  • Lead community engagement initiatives that enhance the organization’s visibility and trust within diverse communities.
  • Plan, organize, and execute community events, including: Senior expos and health fairs, Doc Talks/Lunch & Learns, Sponsorships, Astrana hosted events (fun social events), SWAG Center Activities.
  • Track lead generation from grassroots activities and ensure timely follow-up to convert leads into members.
  • Maintain monthly event invite email blast through Campaign Monitor or another platform.
  • Collaborate with team to develop and implement outreach and marketing strategies tailored to senior population.

Requirements

  • High school diploma or equivalent - bonus to have an Associate's or Bachelor’s degree in Healthcare Administration, Social Work, Public Health, or a related field preferred.
  • 1–3 years of experience in patient engagement, care coordination, or healthcare customer service.
  • Strong interpersonal and communication skills with the ability to build rapport and trust with diverse patient populations.
  • Familiarity with healthcare systems, medical terminology, and insurance navigation (Medicare and Medicaid knowledge is a plus).
  • Proficiency in EHR systems and administrative platforms; ability to document accurately and manage multiple systems simultaneously.
  • Demonstrated ability to manage time effectively, multitask, and problem-solve in a fast-paced environment.
  • Passion for patient-centered care and improving health outcomes in underserved communities.

Preferred

  • Bilingual (Spanish or other languages) preferred but not required.
