Job Description

Responsibilities

Source, screen, and close new case referrals through grassroots community outreach.
Build relationships with community leaders and individuals to establish trust and promote services.
Organize and represent the company at various community locations (churches, supermarkets, senior homes, food pantries, schools, cultural events, rehabs, hospitals, etc.).
Serve as the primary point of contact for new cases and provide support to administrative staff.

Requirements

Proven experience in marketing/community outreach, preferably in the healthcare or senior care industry
A valid driver's license and personal vehicle are required
Bilingual in English & Spanish required

Preferred

Home Care experience preferred
Knowledge and understanding of Medicaid waiver eligibility and enrollment process preferred
