Job Summary
Own community outreach and referral network building for a NJ home care agency, focusing on hospitals, social workers, cultural liaison work, and bilingual caregiver recruitment; turn outreach into sustained referrals while educating families on Medicaid/private pay and care access.
- Growth & Partnerships: Develop and maintain referral relationships with hospitals, rehab centers, discharge planners, and social workers.
- Community Education: Educate families and partners on home care services, Medicaid/private pay options, and how to access care.
- Cross-Cultural Outreach: Serve as a cultural liaison to improve communication across diverse communities and track outreach performance.
Job Description
- Develop and maintain referral relationships with: Hospitals, rehab centers, discharge planners, and social workers
- Senior centers, adult day programs, and case managers
- Cultural organizations, religious institutions, and community leaders
- Represent the agency at: Health fairs, senior expos, and community events throughout NJ
- Drive client referrals and caregiver recruitment, especially bilingual Home Health Aides (HHAs)
- Educate families and community partners on Home care services, Medicaid and private pay options, How to access care
- Serve as a cultural liaison to support communication across diverse communities
- Track outreach activity, referral sources, and performance metrics
- Experience in home care, healthcare outreach, or senior services
