Job Summary

Own the admissions funnel in a long term care setting by coordinating referrals, verifications, tours, and transitions, while aligning hospitals and families with cross-functional teams and regulatory requirements.

  • Admissions workflow ownership: coordinate referrals, insurance verification, tours, and resident transitions.
  • Stakeholder orchestration: communicate with hospitals, families, case managers, discharge planners, and the internal care team.
  • Compliance and census focus: track referrals and admission outcomes, ensure regulatory adherence, and support census development.

Job Description

Responsibilities

  • Coordinate all aspects of the admissions process from referral through admission
  • Respond promptly to hospital, family, and community inquiries
  • Verify insurance coverage and obtain necessary documentation
  • Communicate effectively with interdisciplinary team members regarding incoming admissions
  • Conduct tours for prospective residents and families
  • Maintain relationships with hospitals, case managers, discharge planners, and referral sources
  • Track referral activity and admission outcomes
  • Ensure compliance with state and federal admission regulations
  • Assist with marketing and census development initiatives
  • Support residents and families during the transition into the facility

Requirements

  • Previous experience in healthcare admissions, marketing, social services, or customer service preferred
  • Skilled nursing facility or long-term care experience strongly preferred
  • Excellent communication and interpersonal skills
  • Strong organizational and multitasking abilities
  • Knowledge of Medicare, Medicaid, and insurance verification processes preferred
  • Ability to work collaboratively with residents, families, hospitals, and internal teams
  • Proficiency with electronic medical records and Microsoft Office applications
