Job Summary

Own local census growth through hands-on community outreach, hospital visibility, and referral partnerships that feed admissions, then back it with structured tours, post-discharge follow-ups, and reputation-building work.

Community & Partnerships: Build referral networks and represent the facility at community touchpoints and with partner hospitals.

Presence & Reputation: Manage social channels, content capture, and online reviews to keep the facility top of mind in the community.

Maintain a consistent schedule of in-person visits to Assisted Living and Board & Care homes within the service area
Build and nurture referral relationships in partnership with the Discharge Planning and admissions teams
Represent Hemet Hills Post Acute professionally as the face of the facility in the community
