We are a regional full service senior health care provider operating in western Ohio encompassing 13 facilities. We serve over 800 nursing home residents and have an additional 300 assisted living apartments. We provide a seamless continuum of care by utilizing our integrated affiliates. Therapy Solutions provides therapy (Physical, Occupational and Speech) in our facilities as well as in the home. Fountain Park Home Care and Home Health Care Solutions provides personalized one on one care in the comfort of your home. We also provide medications through Pharmacy Solutions.
