Leading Home Care is a home and community-based care provider serving individuals with disabilities, the aging, and their families. Our clients rely on our personal caregiving services to live independently and maximize quality of life. The Registered Nurse will impact lives every day by monitoring medical care, medications, treatments, and services for approximately 20 clients located in Baton Rouge and the surrounding areas within Region 2, as well as providing staff training. This is a non-clinical 40-hour/week role with office hours of 8:30 a.m. – 4:30 p.m. Monday through Thursday and 8:30 a.m. – 2:00 p.m. on Friday. The job is performed primarily in-office but will require occasional visits to client homes. A company vehicle is available for travel. RN Responsibilities: * Monitor medical care, medication, and non-complex task administration for approximately 20 clients * Perform medical assessments for potential and/or current clients to determine if LHCL is capable of providing appropriate services * Conduct home visits bimonthly or as needed based on the client's needs & program * Host weekly Medication Administration Training class for Direct Support Workers * Ensure Direct Support Workers (DSWs) are appropriately trained and competent to administer medication and perform non-complex tasks, providing re-training when necessary * Delegate and provide direction on medication administration and non-complex tasks to DSWs, communicating updates as medications are changed by licensed healthcare provider * Monitor Medication Administration Records (MARs) completed by DSWs * Communicate frequently with administrative staff, case managers, doctor’s offices, clients, families, and staff to coordinate necessary medical care, medications, and treatments * Be organized and consistent with proper documentation procedures * Comply with all licensing and program guidelines established by LA Department of Health, Office for Citizens with Developmental Disabilities, Office for Aging and Adult Services, and Medicaid * Promote and market LHCL in the local community within the permissible standards * Establish and maintain a professional and open line of communication with affiliates of LHCL * Model the LHCL values of Compassion, Trust, Integrity, and Accountability * Additional related tasks as needed Requirements: * Must have a valid Louisiana RN License * 2+ years’ related experience in a clinical or administrative Nursing environment is preferred * Some after-hours availability for occasional questions or issues regarding client medication or health status * Proficient computer skills including Microsoft Office (Outlook, Excel, Word) and ability to operate office equipment * Must possess strong administrative and clinical skills. * Must be detail-oriented, communicate effectively and have strong prioritization and time management skills * Ability to interact professionally with LHCL clients and their families, employees and representatives of other community-based services that support LHCL and our recipients * Valid driver’s license, auto insurance, and reliable transportation * Must be able to read, write, and follow instructions in English * Must satisfactorily pass all required and applicable State & Federal eligibility verifications including background, driving, and other record checks * Physical requirements include stooping, kneeling, reaching, standing, walking, and climbing stairs. Must be able to occasionally lift up to 50lbs (carrying files, boxes, etc.) _Due to the nature of providing community-based care for people with developmental disabilities, it is necessary that every employee of LHCL must be capable of assisting our clients and LHCL in areas that may be outside the specific elements of this job description.
