The Hartford HealthCare Center for Healthy Aging is a resource and assessment center designed to make it easier for seniors, their loved ones and caregivers to access essential information and services to attain the optimal quality of life.
The Resource Coordinator is the core organizer of the Center for Healthy Aging information, programs and activities. In addition to building and maintaining this knowledge base, the Resource Coordinator will participate in and support the
Center 's mission to connect clients and their families with necessary services to meet their needs and attain optimum quality of life.
Responsibilities include:
  • Assessing and assisting seniors and their families to identify their needs and connect them to resources, including home visits
  • Assisting in education and training programs for caregivers, elders and family members
  • Acquiring and maintaining resource materials (brochures, books, tapes, etc.)
  • Marketing the Center to professionals and referral sources within the Hartford HealthCare System and the community
  • Maintaining liaisons with the seniors, their families, support personnel, etc., to assure that the client’s needs are continually met
  • Communicating with physicians(s) and other health care providers for the client in accordance with HIPAA requirements
  • Building and maintaining positive relationships in the community with hospitals, social service agencies, physicians and other health care providers, senior centers, community and professional groups and other possible referral sources as well as influential individuals and professionals of the community at large
  • Processing referrals to the Center and assisting seniors in transitions between levels of care and providers of services
  • Coordinating visits, meetings, presentations and special events related to center activities
  • Collecting, entering, managing and reporting data related to referral patterns
  • Assists in curriculum development, policies, strategic planning and procedures of the Center
  • Performs other related duties as assigned or necessary

Qualifications


  • Bachelor’s degree in Human Services or related field
  • 3 years’ experience in case management/discharge planning or working with older adults
  • Proficient in Microsoft Office applications (e.g., Word, Excel, Access, PowerPoint)
  • Exceptional customer service skills
  • Exceptional communication skills
  • Ability to work independently and productively in a high volume, multifaceted work environment
  • Detail oriented with an ability to manage multiple task assignments in a highly organized fashion
  • Possesses a commitment to holistic care in meeting the needs of clients.
  • Valid CT driver’s license and the ability to travel independently throughout the community served
