Job Summary

Own the community-facing hospice partnership efforts and drive patient referrals by building relationships with providers and educating families.

Develop and maintain strong ties with doctors, nurses, social workers, and discharge planners to seed referrals.
Educate families on hospice benefits, eligibility, and the referral process to streamline service start.

Job Description

Responsibilities

Visit hospitals, nursing homes, assisted living, and clinics.
Maintain strong relationships with doctors, nurses, social workers, and discharge planners.
Represent the hospice professionally in the community.
Provide education about hospice benefits and eligibility.
Answer common questions about insurance, medications, equipment, and eligibility.
Coordinate with admissions, nursing, and the office team to start services.
Explain the referral process to families and providers.

Requirements

Bachelor's Degree in Marketing.
Must be organized and detail-oriented.
Must be able to communicate effectively orally and in writing.
