Job Summary

Own regional outreach to build a referral network, drive private pay referrals, and onboard seniors with clear tracking in Wellsky.

  • Own referral-network development with healthcare professionals and community partners in designated zip codes.
  • Drive private pay referrals via outreach including calls and after-hours availability to meet weekly KPI targets.
  • Ensure seamless onboarding and accurate activity logging in Wellsky to sustain pipeline visibility.

Job Description

Responsibilities

  • Aim to hit a monthly quota of 4-8 new client starts.
  • Identify and establish relationships with potential clients and referral sources within the designated zip codes.
  • Get private pay referrals: Conduct outreach activities, including calls and meetings, with availability during evenings and weekends as needed for prospect calls.
  • Ensure seamless onboarding of new clients.
  • Maintain accurate records of outreach activities and client interactions using Wellsky, document leads and referral sources developments.

Requirements

  • Minimum of 2–5 years of experience in marketing, business development, community outreach or a similar role (home care experience preferred).
  • Proven ability to build and maintain relationships with healthcare professionals, referral sources, and community partners.
  • Demonstrated success in meeting or exceeding sales, referral, or business development goals.
  • Excellent communication, presentation, and interpersonal skills.
  • Comfortable representing the company at networking events, community meetings, and healthcare facilities.
  • Proficient in Microsoft Office and other basic office software.
  • Professional, self-motivated, and results-driven with a passion for helping seniors remain safe and independent at home.
  • Ability to work independently and manage a flexible schedule while achieving weekly KPI's (assessments and APFM leads may come outside of a 9 to 5)
  • Valid driver's license, reliable transportation, and willingness to travel within the service area.

Preferred

  • Experience with Wellsky software for tracking outreach and client interactions.
  • Familiarity with funding alternatives VA&A, VA, and LTCi.
  • Experience in home care, healthcare, or senior services.
  • Knowledge of the Corpus Christi healthcare community.
  • Bilingual (English/Spanish) is a plus.
