Job Description
Social Worker - Healthcare Services
We are seeking a compassionate and dedicated Social Worker to join our healthcare team at DFCC. In this vital role, you will provide essential social work services to patients, supporting their emotional, social, and economic well-being. Your expertise will help ensure a holistic approach to patient care, fostering collaboration across clinical teams and external agencies to deliver comprehensive support.
Key Responsibilities
- Facilitate, plan, and organize social work services for patients presenting at DFCC.
- Collaborate with clinical staff to identify patient and caregiver needs, promoting continuity of care and services.
- Work with external agencies and providers to coordinate social work, case management, and entitlement services.
- Demonstrate competency in delivering social work services based on the medical care plan, considering psycho-social, economic, cultural, ethnic, religious, and individual patient factors.
- Organize and facilitate support groups and individual counseling sessions tailored to patient needs.
- Maintain accurate documentation of patient interactions and service plans.
- Advocate for patients’ rights and connect them with appropriate community resources.
Requirements
- Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited program.
- Valid licensure or certification as required by local regulations.
- Minimum of one year experience in healthcare social work or a related setting.
- Strong understanding of psycho-social assessment, case management, and resource coordination.
- Excellent communication, interpersonal, and counseling skills.
- Ability to work independently and collaboratively with multidisciplinary teams and external agencies.
- Cultural competence and sensitivity to diverse patient populations.
- Ability to handle sensitive situations with professionalism and empathy.
- Computer literacy with proficiency in Microsoft Word. Experience documenting in an EMR
- Experience with chemical dependence, Medication Assisted Treatment (MAT), and HIV treatment preferred.
- Bi-lingual skills in English and Spanish are preferred.
