Job Description
Summary
Responsible for all aspects of the referral and intake process to Broaddus Hospital and Mansfield Place. Receives and coordinates all referrals for admissions to Broaddus Hospital and Mansfield Place. Provides education on clinical documentation to support severity of illness and intensity of service. Facilitates the efficient delivery of quality patient care in the most effective manner by managing resources.
Responsibilities
Core Duties
Receives and coordinates all referrals for admission to Broaddus Hospital and Mansfield Place. Independently reviews referrals and makes initial determination as to appropriateness of admission to swing bed, Mansfield Place, acute care and observation.
Gathers and reviews all clinical patient information related to the referral and forwards information to the interdisciplinary team for further review as to the specific care to be provided to the patient.
Reviews patient clinical information and compares various insurances admission criteria to qualify patient for admission to the appropriate unit of Broaddus hospital/Mansfield Place.
Performs initial and recurrent psychosocial interviews and evaluations. Assists in the development of individual psychosocial treatment plans.
Participates as a member of the transitional team, coordinates care planning and discharge preparation and planning, including outgoing referrals to agencies, SNF, ALF, ERC, HH/Hospice. Conducts daily patient huddle.
Prepares and maintains required documentation on each patient. Assists new admissions with patient’s rights, advanced directives and Power of Attorneys. Completes new admission paperwork, patient interviews and documentation.
Job-Specific
As needed, performs discharge planning for appropriate care and communicates information to primary care physician as needed. Assists providers and family with the referral process. Orders DME.
Communicates and provides education on supporting documentation to provider and nursing team ensuring a quality medical record.
Conducts utilization processes with insurance carriers to obtain required pre-certification and pre-authorization of admissions and expected length of stay and initial pre-certification and pre-authorizations for Mansfield Place. Continued authorization for swing bed, inpatient and observation stays.
Reviews charts for appropriate choice of observation verses inpatient admission, and to ensure proper forms are present. Communicates information to providers and health care team with help of the Unit Manager.
Facilitates peer meetings when needed for insurance denials on inpatient and observation patients. Keeps a log of denials and forwards as needed to Davis Medical Center Director of Case Management.
As needed, coordinates community referrals to home health, nursing homes, assisted living, APS, etc.
Reviews and logs of all incoming swing bed and Mansfield Place referrals and communicates with referring facilities. Maintains waiting list for Mansfield Place referrals and follows up with potential new admissions as beds become available. Coordinates admissions with appropriate Mansfield Place Staff and Social Worker.
Visual monitoring of cardiac monitors as needed.
Coordinates and participates in patient/family meetings as necessary.
Consults in ED Regarding potential admissions.
Reviews charts to ensure proper forms are present, such as; MOON letter, Important notice from Medicare, Notice of Discharge, etc.
Meets with and communicates with other facilities and entities regarding bed availability and admission process. Participates in public relations and marketing activities to promote services provided by the facility.
Maintains established departmental policies and procedures, objectives, supports quality assessment and performance improvement processes, adheres to safety, environmental and infection control standards.
Maintains professional growth and development through seminars, workshops and literature.
Demonstrates the knowledge and skills necessary to provide social services care appropriate to the age of the patient ranging from newborn to geriatric.
Attends meetings as required and participates on committees as directed.
Performs other duties as assigned.

Education and Experience Requirements
Education
Minimum education: Associate degree
Preferred Education: Bachelor’s Degree
Experience
Experience in LTC, Swing bed, and Acute Care admission criteria
Good writing skills to clearly articulate information
Great human relations and oral communications skills. Good problem-solving skills.
Credentials
Current WV Social Worker license or
Current WV RN license
BLS
Working Conditions
Monday-Friday 7:30-4:00 or 8-4:30
Weekends off
Approved Vandalia Holidays off.
Competencies and Skills
Training in agency and regulatory requirements.
Clinical expertise in the specialty is preferred.
Successful completion of telemetry monitor class or certification.
