Job Summary
Own local outreach to hospitals and community partners to drive referrals and census growth while representing the agency in NYC markets.
- Develop and nurture referral networks with hospitals, nursing homes, physician offices, and community orgs to maintain a steady pipeline.
- Lead outreach events and presentations to raise awareness and enroll new clients.
- Maintain proactive, ongoing partner communications and reliable enrollment support to ensure strong partnerships.
Job Description
Own local outreach to hospitals and community partners to drive referrals and census growth while representing the agency in NYC markets.
- Build and strengthen relationships with hospitals, nursing homes, physician offices, rehabilitation facilities, adult day health programs, social workers, case managers, and community organizations.
- Develop referral sources and promote services through community outreach, events, and presentations to increase awareness and enrollment.
- Manage enrollment support, timely follow-ups on referrals, and ongoing partner communications to sustain partnerships and high service levels.
