Job Summary

Own the Houston referral network by turning hospitals, clinicians, and senior-care partners into steady private-pay referrals while building durable partnerships.

  • Grow and manage referral partnerships with hospitals, case managers, social workers, discharge planners, and other care providers.
  • Educate healthcare professionals about Bloom Home Care services to drive referrals.
  • Track referrals and marketing activity to show pipeline and outcomes.

Job Description

Responsibilities

  • Develop and maintain referral relationships with Hospitals, Case managers, Social workers, Discharge planners, Skilled nursing facilities, Rehabilitation hospitals, Assisted living communities, Independent living communities, Memory care facilities, Physicians, Geriatric care managers, Elder law attorneys, Senior living advisors, Concierge medical practices, Community organizations
  • Educating healthcare professionals about Bloom Home Care services
  • Building long-term referral partnerships
  • Identifying prospective private-pay clients
  • Following up with referral sources
  • Representing Bloom Home Care professionally at networking events
  • Tracking marketing activities and referrals

Requirements

  • Experience generating referrals for home care, hospice, home health, assisted living, rehabilitation, or other healthcare services
  • Previous healthcare marketing experience
  • Existing referral relationships within the Houston healthcare community
  • Self-motivated and independent
  • Reliable transportation
  • Comfortable traveling throughout the Houston metropolitan area

Preferred

  • Existing referral relationships within the Houston healthcare community
  • Experience marketing: Non-medical home care, Home health, Hospice, Senior living, Rehabilitation services
  • Strong communication and relationship-building skills
  • Self-motivated and independent
  • Reliable transportation
