Job Summary
Own the local hospital and referral-source network to smooth transitions and discharge planning while collaborating with physicians and social workers.
- Growth & Partnerships: Build relationships with acute-care facilities, discharge planners, and referral sources to drive transitions.
- Marketing & Strategy: Develop and execute a local marketing strategy with regional teams, create materials, and run market assessments.
- Community & Leadership: Lead strategic planning, identify service opportunities, and engage in community events with substantial travel.
Job Description
Responsibilities
- Responsible for building and maintaining relationships with acute-care hospitals, health systems, discharge planners, case managers, rehabilitation centers, and assisted living facilities
- Work with physicians and hospital discharge planners to arrange a smooth transition to the facility
- Build and maintain relationships with hospitals, physicians, social workers, discharge planners and other referral sources
- Help develop and implement a marketing strategy for the facility with our regional marketing team as well as managers in the field
- Develop marketing materials such as brochures, flyers, ads, campaigns etc.
- Continuously conduct market assessments and develop a comprehensive marketing plan designed to meet company goals and objectives
- Participate in community events and activities that promote company goals and initiatives
- Provide leadership in strategic planning including identifying opportunities for additional or improved services to address customer needs
- Approximately 70% local travel is expected for this position
