Job Summary

Own and grow the agency\'s referral network by forging and maintaining relationships with hospitals, physicians, rehab centers, and senior services; drive referral opportunities through community outreach, events, and health fairs; partner with the team to ensure smooth client admissions and excellent service.

Build and maintain a broad referral network across hospitals, physicians, rehab centers, and senior services to drive client referrals.
Lead outreach, presentations, networking, and events to identify opportunities and ensure smooth admissions and exceptional service.
Track referral activities, meet goals, and use basic computer tools to stay organized; hold valid license to enable field work.

Job Description

Responsibilities

Develops and maintain referral relationships with hospitals, physicians, rehab/SNF centers, assisted living communities, senior centers and other healthcare providers
Promotes the agency's homecare services through community outreach, presentations, networking events and health fairs
Identify new referral opportunities and implement strategies to increase client referrals
Maintain accurate records of referral activities and marketing efforts
Collaborate with the team to ensure smooth client admission and exceptional customer service
Represent the agency professionally while uploading privacy and regulatory requirements

Requirements

Strong networking, relationship building and communication skills
Self motivated with the ability to work independently and meet referral goals
Valid driver's license
Proficiency in basic computer software

Preferred

Experience in home healthcare marketing, hospice, or community outreach is preferred
