Own and grow organizational referral networks by actively managing community partnerships, conducting targeted outreach, and coordinating with clinical teams to ensure seamless access to services.

Job Description

Responsibilities

  • Develop, maintain, and strengthen relationships with community referral sources, including hospitals, healthcare providers, behavioral health agencies, social service organizations, courts, probation offices, veteran organizations, and community partners.
  • Conduct routine in-person outreach visits to referral partners to educate them on available services and maintain strong working relationships.
  • Identify and develop new referral opportunities within assigned service areas.
  • Represent the organization at net»vorking events, community meetings, conferences, health fairs, and other outreach opportunities.
  • Coordinate, schedule, and facilitate educational presentations, lunch-and-learns, and community trainings.
  • Promote organizational services while maintaining a thorough understanding of all programs, admission criteria, payer requirements, and service offerings. Collaborate closely with
  • Admissions and Clinical teams to ensure seamless referral coordination and continuity of care. Monitor referral trends and identify opportunities to improve community engagement and organizational growth.
  • Maintain accurate documentation of outreach activities, referral contacts, meetings, and follow-up communications within the CRM or other dedicated platforms.
  • Ensure timely follow-up with referral sources regarding pending referrals while maintaining confidentiality and HIPAA compliance.
  • Assist in developing and implementing strategic outreach initiatives to expand market presence. Coordinate community partnerships that support client access to healthcare, housing, employment, education, transportation, and other supportive resources.
