Wellness WoRx Foundation is a community-centered health organization dedicated to bridging gaps in access to care for underserved populations. Through education, navigation, and strategic community partnerships, we connect people to the resources and providers they need to manage health conditions — including dermatologic care. Our model is built on trust, local infrastructure, and the belief that the right community connector can change outcomes at scale.

THE OPPORTUNITY

We are looking for a Lancaster-based community strategist who is already embedded in this community — not someone who needs to build relationships from scratch. You know the landscape, you know the players, and you know how to move people to action. This is a plug-and-play role for someone who can hit the ground running from day one.

You will lead all community engagement activity for the Wellness WoRx Foundation's skin health initiative in Lancaster — mapping and activating partner organizations, co-creating programming, standing up monthly access point events, and building the connective tissue that routes community members toward education, navigation, and care. You are the program on the ground.

WHAT YOU'LL DO

Community Mapping & Partner Activation

  • Identify and prioritize 10-12anchor partner organizations in Lancaster with existing community reach to support dermatologic care, including: FQHCs, faith communities, barbershops, salons, advocacy groups, and community centers
  • Train community health workers and trusted intermediaries as skin health navigators who can extend your reach into their own networks
  • Build and maintain a shared directory of Medicaid-accepting providers and community care resources in the Lancaster area
  • Conduct regular direct community outreach in everyday local spaces â€” barbershops, salons, laundromats, church steps, community events — showing up consistently enough to become a trusted, recognizable presence in the neighborhoods you serve

Programming & Event Execution

  • Design and execute a recurring monthly community access event calendar — minimum two permanent monthly events, with a goal of multiple sites across Lancaster
  • Coordinate event logistics, including venue setup, materials, registration, giveaways, and partner staffing
  • Co-create skin health education materials— ensuring cultural relevance, plain-language clarity, and practical utility
  • Bring device and internet access to events, enabling community members to connect to virtual care resources on-site
  • Execute informal pop-up activations independently â€” tabling after a church service, showing up at a local market, joining an existing community gathering — and convert those moments into education conversations, navigation support, and care referrals

Navigation & Care Connection

  • Provide one-on-one navigation support at events — walking individuals through insurance questions, care access pathways, and appointment scheduling
  • Execute warm handoffs to clinical partners, including telehealth and FQHC-based providers, ensuring community members follow through on care connections
  • Track every navigation interaction and maintain accurate records of individuals reached, connections made, and follow-up status

Strategy, Measurement & Reporting

  • Own your city's performance metrics â€” events hosted, individuals reached, navigator interactions, care referrals, and appointment completion rates
  • Submit biweekly activity reports to the Program Director with quantitative data and qualitative field observations
  • Identify gaps, barriers, and opportunities in real time and bring forward recommendations to adjust tactics quickly
  • Contribute to quarterly program reporting shared with foundation leadership and program sponsors

WHO YOU ARE

You are not a generalist who can figure Lancaster out over time. You are someone who is already woven into the fabric of this community — whose phone gets answered, whose invitation gets accepted, who knows which organizations have real reach and which ones don't. But you are also the person community members themselves will walk up to and trust. You move between organizational relationships and direct community presence with equal ease, and you understand that real impact happens in both rooms.

You bring:

  • Deep existing relationships in Lancaster PA â€” with community organizations, faith networks, FQHCs, social service providers, or public health infrastructure
  • Demonstrated ability to activate quickly â€” you've launched programs, events, or initiatives before and know how to go from idea to execution in weeks, not months
  • Strategic thinking paired with tactical execution â€” you can see the big picture and also show up, set up, and run the event yourself
  • Health navigation or community health experience â€” you understand how underserved populations access care, and you can speak to insurance, referrals, and barriers without hesitation
  • Strong organizational and tracking discipline â€” you document your work, hit your numbers, and take data seriously as a measure of real impact
  • Comfortable with ambiguity and autonomy â€” this is not a heavily supervised role; you are expected to own your city and drive results independently
  • Bilingual English/Spanish is a strong plus
  • Community Health Worker or Social Worker certification or experience preferred
  • Personal trust and credibility with everyday community members â€” not just organizational leaders. You are someone people will approach at a block party, a barber chair, or outside a church — and actually listen to and act on what you share

WHAT SUCCESS LOOKS LIKE AT 30 DAYS

  • 6+ active partner organizations are formally engaged with co-programming underway
  • Minimum 2 recurring monthly access point events established and running at consistent sites
  • 500+ community members reached through education sessions, events, or one-on-one navigation interactions
  • Warm referral pathway established with at least three FQHCs and one telehealth derm provider (Medicaid accepting) serving Lancaster
  • Real-time tracking system in place with biweekly reporting delivered on schedule

ENGAGEMENT TERMS

This is an independent contractor engagement. The Community Engagement Strategist will be responsible for their own taxes and does not receive employee benefits. The contractor will invoice bi-weekly based on hours worked at $35.00/hour, not to exceed 20 hours per week without prior written approval from the Program Director.

WORK ENVIRONMENT

This is a field-first role. Approximately 80% of your working hours will be spent out in the community — at partner sites, pop-up events, community gatherings, and direct outreach activations across Lancaster. The remaining time (~20%) involves administrative work such as reporting, scheduling, partner communications, and documentation, which you will perform remotely from a home office maintained at your own expense.

Wellness WoRx Foundation will provide a Wi-Fi-enabled company tablet for use in the field and for program-related tasks. Contractors are responsible for their own internet connectivity, home office setup, and any personal equipment beyond the provided tablet.
