Responsibilities

• Source, screen, and close new case referrals through grassroots community outreach.
• Build relationships with community leaders and individuals to establish trust and promote services.
• Organize and represent the company at various community locations (churches, supermarkets, senior homes, food pantries, schools, cultural events, rehabs, hospitals, etc.).
• Serve as the primary point of contact for new cases and provide support to administrative staff

Requirements

• Bilingual in English & Spanish required
• A valid driver's license and personal vehicle are required
• Proven experience in marketing/community outreach, preferably in the healthcare or senior care industry
• Energetic, organized and able to work independently, have strong interpersonal skills, be self motivated and results driven

Preferred

• Home Care experience preferred
• Knowledge and understanding of Medicaid waiver eligibility and enrollment process preferred
