Job Description

Responsibilities

  • Referral Development & Relationship Management: Build and sustain strong relationships with acute‑care hospitals, healthcare systems, discharge planners, case managers, rehabilitation centers, assisted living facilities, hospices, and home care agencies
  • Referral Development & Relationship Management: Maintain ongoing communication with physicians, social workers, and other referral sources
  • Referral Development & Relationship Management: Collaborate closely with hospital discharge planners and physicians to ensure smooth patient transitions
  • Admissions & Clinical Support: Maintain responsibility for patient referrals through completion of the admission process
  • Admissions & Clinical Support: Demonstrate a strong understanding of inpatient rehabilitation and LTACH admission criteria
  • Admissions & Clinical Support: Conduct on‑site pre‑admission assessments as needed
  • Admissions & Clinical Support: Utilize fast‑track referral processes to support timely conversions
  • Marketing & Community Engagement: Partner with regional marketing teams and facility leadership to develop and implement effective marketing strategies
  • Marketing & Community Engagement: Create and support marketing materials, including brochures, flyers, advertisements, and campaigns
  • Marketing & Community Engagement: Participate in community events and outreach activities to support organizational growth and brand awareness

Requirements

  • LPN or RN license preferred
  • Minimum of two (2) years of experience in the licensed field, ideally within rehabilitation, LTACH, or a related post‑acute setting
  • Prior marketing or business development experience, preferably in senior living, skilled nursing, or healthcare
  • Strong interpersonal, communication, and relationship‑building skills
  • Ability to engage physicians, healthcare professionals, and community partners professionally and tactfully
