As a Care Coordinator with FirstLight, you’ll work closely with caregivers and clients to establish quality assurance of services. You will manage areas of daily operations which include the supervision of client services and caregivers. You will directly interface with clients and family members.

You will also aide in building meaningful relationships with referral partners, healthcare providers, and community organizations while networking in the community.

What You’ll Do

  • Conduct care consultations with prospective clients and families
  • Manage the client intake process
  • Facilitate client/caregiver introductions with every new client and with every new caregiver
  • Perform initial and ongoing in-home evaluations
  • Manage the hiring process for caregivers and conduct orientation/onboarding
  • Work closely with caregivers to provide support, coaching and ongoing caregiver training
  • Proactively identify, qualify, and develop relationships with referral sources in healthcare, senior living, and the community.
  • Conduct in-person visits, presentations, and networking events to promote FirstLight’s services.
  • Represent FirstLight at community events, health fairs, and industry gatherings.

  • You have a high school diploma or general education degree (GED); and two (2) years related home care/home health care industry experience and/or training; or equivalent combination of education and experience.

The candidate for this position must possess and maintain and Valid Driver's License, automobile insurance, Level 2 Background check, and CPR certification.

As a percentage of work will occur outside of the office, the candidate must also have a reliable source of transportation.

A monetary auto-reimbursement will also be issued on a bi-weekly basis to compensate for travel in addition to the base salary.

This is a full-time, salaried position in which a minimum of 40-hr in-office work week is required Monday-Friday from 8am-4pm to include a rotating on-call schedule.

Employee benefits are offered within 60-90 days of employment.
