Job Description

Position Summary
The Chief Medical Officer (CMO) is the senior physician executive responsible for enterprise clinical governance, physician strategy, resident safety, quality, regulatory excellence, physician services, and clinical innovation across a growing skilled nursing enterprise. Reporting to the Chief Executive Officer, the CMO initially oversees four facilities before expanding to enterprise responsibility across a 40-facility portfolio while preserving the statutory responsibilities of each facility Medical Director, Administrator, Governing Body, Director of Nursing, and Compliance Officer.
Reporting Relationships
  • Reports directly to the Chief Executive Officer.
  • Partners with the COO, CNO, CFO, Chief Ethics & Compliance Officer and
Governing Body.
  • Provides executive oversight of Facility Medical Directors, attending physicians, and advanced practice providers.
Executive Responsibilities
  • Lead enterprise clinical strategy.
  • Standardize evidence-based care.
  • Oversee physician recruitment, credentialing, privileging, peer review and succession.
  • Lead QAPI, infection prevention, resident safety and emergency preparedness.
  • Improve quality, survey readiness and value-based performance.
  • Represent the organization with hospitals, payers and regulators.
Clinical Governance
  • Establish enterprise medical policies.
  • Maintain Medical Director governance model.
  • Lead serious event review and root-cause analysis.
Compliance & Risk
  • Partner with Compliance while preserving independence.
  • Promote documentation integrity and medical necessity.
  • Support CMS, NYDOH, Medicare and Medicaid compliance.
Authority
  • Issue interim clinical directives.
  • Require physician performance review.
  • Recommend privilege restriction through governance.
  • Escalate unresolved risks to CEO and Board.
Qualifications
  • MD/DO with unrestricted NY license.
  • Board Certified; CMD preferred.
  • Executive multi-site SNF leadership.
  • Expertise in CMS, NY regulations, QAPI and value-based care.
First-Year Deliverables
  • 90 days: enterprise assessment.
  • 180 days: physician scorecards and dashboards.
  • 365 days: measurable quality and physician improvements.
