Ledgewood Rehabilitation & Skilled Nursing Center | Massachusetts
Ledgewood Rehabilitation & Skilled Nursing Center is seeking a Case Manager to join our team. This role is ideal for an RN/LPN or SW who enjoys collaboration, critical thinking, and playing a key role in driving strong patient outcomes.

- Coordinate ongoing assessments of patient clinical status and level of care
- Evaluate and adjust care plans to ensure realistic, outcome-driven goals
- Collaborate with patients, families, interdisciplinary teams, and payors
- Manage resource utilization including services, consults, and length of stay
- Coordinates all aspects of discharge planning process and transitions of care
- Track and analyze outcomes data to improve care delivery
- Serve as a resource for staff on managed care and regulatory guidelines
- Maintain accurate, timely, and compliant documentation
- Active Massachusetts LPN, RN or Social Work license required
- 1+ year experience in two or more of the following:
- Case management
- Skilled nursing or sub-acute care
- Discharge planning
- Managed care / Medicare
- Strong understanding of managed care and reimbursement
- Excellent communication and critical thinking skills
- Ability to work independently and collaboratively
