Patient Care Liaison Skilled Nursing Facilities | Tennessee Market Annual salary ranges from $65-$75,000 per year with a healthy incentive plan Position Summary We are seeking an experienced Patient Care Liaison / Business Development Professional to drive strategic referral relationships, census growth, and market expansion across our Skilled Nursing Facilities in the Tennessee market. This role is responsible for cultivating strong partnerships with hospitals, physicians, case managers, payors, and community referral sources while also supporting mentorship and coaching of business development staff to strengthen overall market performance. The ideal candidate is a relationship-driven professional with a strong understanding of post-acute care, referral management, and healthcare sales strategy. Key Responsibilities Strategic Relationship Development Develop and maintain strong referral relationships with hospitals, case management teams, physicians, and community partners Serve as the primary liaison between referral sources and facility clinical/operations teams Identify opportunities to expand strategic accounts and preferred provider relationships Represent the organization at community outreach events, healthcare networking events, and industry meetings Census Growth & Market Development Drive skilled nursing facility census growth through targeted business development strategies Execute market growth plans for assigned territory Monitor referral trends, conversion rates, and admission outcomes Identify opportunities to improve hospital partnerships and referral pipelines Admissions & Clinical Collaboration Work closely with Clinical Teams to ensure appropriate patient placements Facilitate communication between hospital discharge planners and facility leadership Promote awareness of clinical capabilities, specialty programs, and outcomes Mentorship & Team Development Collaborate with regional leadership on sales strategy and performance improvement Share best practices related to relationship management and referral conversion Market Intelligence Provide leadership with insights regarding growth opportunities and challenges Utilize CRM systems to manage accounts, document activities, and track performance metrics Qualifications Required 3+ years of healthcare business development or liaison experience Experience in Skilled Nursing, Post-Acute Care, or Healthcare Services Demonstrated success in new business development and relationship management Experience managing strategic referral accounts Strong communication and presentation skills Ability to build trust with clinical teams, hospital partners, and community stakeholders Valid driver’s license and ability to travel within assigned territory Preferred Experience working with hospital discharge planners and case managers Knowledge of Medicare, managed care, and post-acute care referral processes Familiarity with CRM platforms and referral tracking systems Key Competencies Relationship Building Strategic Account Management Healthcare Sales & Market Development Census Growth Strategy Data-Driven Decision Making Collaboration with Clinical Teams Why Join Us Opportunity to influence market growth and patient access to quality post-acute care Collaborative environment with clinical and operational leadership Competitive compensation and benefits package
