Job Summary

Own the social services operations for a skilled nursing facility, supporting the Director with planning, coordinating, and evaluating resident emotional and social care to meet standards and policies.

Social services operations: plan, implement, and evaluate the department to meet resident needs and regulatory requirements.
Regulatory alignment: ensure compliance with federal, state, and local guidelines and facility policies.
Team collaboration: partner with care teams, residents, and families on individualized plans to improve outcomes.

Responsibilities

Social Services Operations: Plan, organize, implement, and evaluate the department to meet resident emotional and social care needs and comply with standards.
Regulatory Compliance: Align with federal, state, and local guidelines and our policies and procedures.
Team Collaboration: Work with the Director to direct the Social Services Department and coordinate with care teams, residents, and families on individual plans.

Requirements

Education/Qualifications: Must possess a high school diploma or equivalent.

Preferred
