Job Summary
Coordinate outreach to seniors and disabled clients, qualify referrals for Adult Medical Day Care, and build relationships with a network of community and referral sources while maintaining organized documentation and collaboration with facility staff.

- Develop relationships with referral sources and present to individuals and groups to grow partnerships.
- Maintain documentation, complete applications, and collaborate with facility staff.
Job Description
Responsibilities
- The primary responsibilities include recruiting seniors and disabled clients for Adult Medical Day Care services and qualifying those in need.
- Build rapport with seniors, disabled clients, and a variety of referral sources, and be comfortable presenting to individuals and groups.
- Maintain organized documentation, complete reports as needed, process facility applications, and collaborate effectively with facility staff.
Requirements
- Bi-lingual: English/Spanish
- Knowledge of NJ Medicaid and MCO's
- Knowledge of the local Senior Buildings and healthcare community in Union and Essex counties including hospitals, short term rehab centers, physician practices, etc.
- Excellent verbal and written communication skills
Preferred
- BA Degree a plus
- 3 plus years marketing experience preferred in adult medical day care or long term care
