Job Summary

Own the community and provider network to drive referrals and smooth patient handoffs, while educating communities about hospice and Home Health. Use events and partnerships to convert outreach into referrals and monitor trends with clinical teams to spot opportunities.

Provider relationships: secure new referral sources through education and outreach.
Represent at events: attend trade shows and community gatherings to raise awareness and referrals.
Clinical collaboration: align with care teams to ensure seamless transitions and data-driven referral opportunities.

Job Description

Responsibilities

Develop and maintain strong relationships with healthcare providers and community partners.
Identify and secure new referral sources through education and outreach.
Represent Blize Healthcare at trade shows, community events, and professional gatherings.
Educate the community about hospice and Home Health care and services.
Monitor and analyze referral and admissions data to identify trends and opportunities.
Collaborate with clinical teams to ensure seamless patient transitions to hospice and Home Health care.

Requirements

Strong communication, interpersonal, and presentation skills.
Proven ability to build and maintain professional relationships.
Sales or marketing experience, preferably in healthcare.
Knowledge of hospice, and Home Health care continuum, services, and regulatory requirements.
Associate’s degree or equivalent experience.
Ability to lift up to 50 pounds as needed.
Reliable transportation for travel within assigned territory.
