Job Summary
Lead and own a home health marketing program targeting hospitals, physicians, and care facilities to grow referrals in the Inland Empire.
- Develop and execute a regional strategy across hospitals, physicians, SNFs, and care networks.
- Build and maintain relationships with discharge planners, wound clinics, and case managers to drive referrals.
- Coordinate with accountable care networks and IPAs to expand partner pathways and referrals in Inland Empire.
