Job Summary

Own community based support delivery with a focus on goal driven, person centered care and strong documentation to keep plans and progress on track.

  • Person centered support: Build relationships, assess goals, and support achievement across home, work, and community.
  • Rights and advocacy: Protect rights, promote self advocacy, and facilitate caregiver relief when needed.
  • Documentation and compliance: Manage plan implementation, progress notes, incidents, and required records.

Job Description

Responsibilities

  • Develop positive relationships with individuals and families and support achievement of personal goals per goal plans in work, home, school, or community settings.
  • Assess hopes and desires and provide supports that facilitate achievement.
  • Protect rights and promote advocacy, providing relief to caregivers as needed.
  • Serve as a resource on community agencies, services, and supports meeting identified needs.
  • Supervise, educate, and monitor individuals in work, home, school, or community settings.
  • Provide one-on-one support to meet emotional, physical, and medical needs.
  • Maintain a safe environment by practicing safety procedures.
  • Facilitate person-centered, effective relationships that promote self-determination.
  • Provide input into assessment and planning processes and development of the individual’s plan.
  • Implement the plan fully including respite, and document as required by policy, completing all required documentation.

Requirements

  • Education: High School Diploma (Required)
  • Certifications: Driver's License (Valid) - USA
  • Experience: Experience working with adults with a mental health diagnosis and/or developmental disabilities – Not Required
