Responsibilities

• Build and maintain relationships with Hospitals, Rehab Centers, Adult Living Facilities, Skilled Nursing Facilities, OB/GYN offices, Maternal Health Providers, Social Workers, Case Managers, and community organizations
• Conduct daily in-person visits and networking activities
• Identify and develop new referral sources
• Respond to and follow up on service inquiries
• Help guide families through service options
• Participate in or support in-home assessments
• Convert referrals into active clients
• Expand our brand in Volusia/Flagler counties
• Conduct presentations, lunch & learns, and community outreach
• Attend and coordinate small events
• Maintain consistent follow-up with referral partners
• Track contacts, referrals, and outcomes
• Maintain CRM or tracking tools
• Meet weekly activity and performance goals

Requirements

• Experience in healthcare marketing, home care, hospice, or related field (preferred)
• Strong networking and relationship-building experience
• Self-starter with the ability to drive business growth
• Ability to pass a Level 2 background check
• Valid driver’s license and reliable transportation
• Comfortable with local travel (daily fieldwork)
• Basic tech proficiency (Microsoft 365, CRM, phone communication)
