Job Summary

Own intake coordination and stakeholder liaison for hospice referrals while protecting quality care through compliance review and education in the community feed.

  • Intake & Referrals: Own the referral intake process and coordination with facilities, physicians, and the admission flow.
  • Compliance & Documentation: Audit charts, complete consent forms, and ensure admin processes are followed.
  • Education & Perception: Educate families about hospice philosophy and track community and patient perceptions of quality care.

Job Description

Responsibilities

  • Ensure the educational needs of the community related to hospice care and services are assessed and met on an ongoing basis.
  • Continuously conduct audit and review of charts to ensure compliance.
  • Establish and monitor community, customer, and patient perceptions of Bristol Hospice as a high-quality provider of hospice services in the community.
  • Maintain comprehensive working knowledge in the field of community needs.
  • Receive hospice referrals.
  • Conference with Clinical Supervisor and/or Director of Patient Care Services to collaborate and schedule intake activity.
  • Follow intake referral processes.
  • Serve as a liaison between facility, physicians, hospitals, skilled nursing facilities, and community and hospice program with coordination of referrals/admission process and services
  • Promptly respond to customer and referral source requests and concerns.
  • Complete consent forms as assigned and indicated.

Requirements

  • Active LVN license in the state of California
  • One to two years of experience in Hospice Sales preferred.
  • Must have excellent observation, verbal and written communication skills, problem solving skills.
  • Must be able to perform prolonged or considerable walking or standing
  • Must be able to lift supplies and equipment
  • Must be able to perform considerable reaching, stooping, bending, kneeling, and/or crouching • Must have visual acuity and hearing.
  • Must be flexible in work hours
  • Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

Preferred

  • One to two years of experience in Hospice Sales preferred.
