Job Summary

Own and expand hospice partnerships by building relationships with physicians, hospitals, and community partners while coordinating marketing efforts and guiding patients through the referral and admission process.

Manage relationships with referral sources and community partners
Lead educational sessions and informal goals of care conversations
Meet performance goals set by leadership

Job Description

Responsibilities

Build and maintain relationships with physicians, hospitals, skilled nursing facilities, discharge planners, and community partners
Plan and coordinate marketing events, educational sessions, and community luncheons
Educate healthcare stakeholders on hospice and palliative care services
Support patients, families, and referral partners throughout the referral and admission process
Provide hospice informational / goals of care conversations with patients and families
Facilitate initial patient consent process
Attend weekly meetings to report successes, share competitor insights, and identify growth opportunities
Meet or exceed performance expectations set by the Director of Business Development and Executive Director leadership

Requirements

LVN, RN, or MSW required
1–2 years of healthcare marketing/sales experience preferred
Minimum of 2 years hospice experience highly preferred
Strong familiarity with local referral partners including:
Hospitals
Physicians
Board & Care (B&C)
Assisted Living Facilities (ALF)
Residential Care Facilities for the Elderly (RCFE)
Skilled Nursing Facilities (SNF)

Preferred

1–2 years of healthcare marketing/sales experience preferred
Minimum of 2 years hospice experience highly preferred
