Role Description This is a full-time, on-site Social Work Coordinator role based in Manhattan, NY. The Social Work Coordinator will provide direct care and support to individuals and families, connect patients with community-based resources, and ensure access to necessary social services. This role involves conducting client assessments, developing care plans, coordinating care, and collaborating with interdisciplinary teams to meet the social and emotional needs of patients. Additional responsibilities include conducting training sessions and providing education for individuals and their families on available resources and support services. Qualifications Strong skills in Social Services, including needs assessment and coordination of resources Experience in Training and facilitating educational programs for clients and team members Ability to provide Teaching and guidance to individuals and families on care plans and available resources Excellent interpersonal and communication skills Strong organizational and problem-solving abilities Master’s degree in Social Work or a related field is preferred Current relevant licensure in New York State (LMSW or higher) is required Experience in healthcare or social service settings is highly desirable
