Outreach Specialist (CHHA)

Location: Saratoga County, NY
Salary: $60,000 – $65,000 annually + mileage reimbursement + benefits
Schedule: Full-Time (Flexible hours, some evenings/weekends for events)

Position Overview

The Outreach Specialist is responsible for developing and maintaining strong referral relationships to support patient census growth for our Certified Home Health Agency (CHHA). This role focuses on community engagement, strategic partnerships, and brand visibility throughout Saratoga County and surrounding areas.

Key Responsibilities

• Build and maintain relationships with referral sources, including:
  • Hospitals & discharge planners
  • Physician offices & clinics
  • Skilled nursing facilities (SNFs)
  • Assisted living facilities
  • Community organizations and senior centers
• Conduct regular in-person visits to referral partners to promote CHHA services
• Educate providers and community partners on home care eligibility, services, and benefits
• Identify new business opportunities and expand referral network
• Coordinate and attend community events, health fairs, and outreach initiatives
• Track and report referral activity, trends, and outcomes using CRM tools
• Collaborate with intake and clinical teams to ensure smooth patient transitions
• Monitor market trends and competitor activity within Saratoga County
• Maintain compliance with all federal, state, and agency regulations

Qualifications

• Bachelor’s degree in Healthcare Administration, Marketing, Business, or related field (preferred)
• 2+ years of experience in healthcare outreach, marketing, or business development (home care or hospice strongly preferred)
• Strong understanding of CHHA services and referral processes in New York State
• Excellent communication, networking, and relationship-building skills
• Self-motivated with the ability to work independently and meet growth targets
• Valid driver’s license and reliable transportation (required)

Preferred Experience

• Existing relationships in Saratoga County healthcare network
• Knowledge of Medicare/Medicaid home care eligibility
• Experience working with CRM systems
