Job Summary
Own and coordinate Vita Nova Home Care's office operations and outreach program, handling daily admin, intake, scheduling, records, and partner development while building relationships with hospitals, clinics, and community organizations across Brooklyn, Queens, and Nassau County to support growth.

Job Description
Responsibilities
- Coordinate the day-to-day administrative functions of the office
- Answer incoming calls and assist clients, families, caregivers, and referral sources
- Assist with new client inquiries and referral intake
- Follow up on prospective clients and referrals
- Maintain accurate electronic and paper records
- Assist with scheduling and caregiver communication
- Prepare correspondence, reports, spreadsheets, and other office documents
- Track outstanding administrative items and ensure timely follow-up
- Communicate with management regarding office operations, client needs, and staffing concerns
- Maintain a professional, organized, and welcoming office environment
Requirements
- High school diploma or equivalent required; associate or bachelor's degree preferred
- Previous administrative, healthcare, home care, medical office, customer service, outreach, marketing, or business development experience preferred
- Excellent verbal and written communication skills
- Strong organizational and follow-up skills
- Professional telephone and interpersonal skills
- Comfortable speaking with healthcare professionals and community organizations
- Comfortable conducting both telephone and in-person outreach
- Basic proficiency with Microsoft Office, Google Workspace, email, and office technology
- Ability to work independently and take initiative
- Reliable transportation and ability to travel locally for outreach activities
