Hospital/Clinical Liaison – Travel Required

Allaire Health Services is seeking an experienced Hospital/Clinical Liaison to join our growing team in Vermont. This is an excellent opportunity for a motivated healthcare professional with strong relationship-building skills and a passion for driving patient referrals and exceptional care transitions.

Territory Includes:

  • Central Vermont

  • Grafton County, New Hampshire

  • Additional secondary territories as needed

Position Highlights:

  • Full-time opportunity

  • Travel throughout the assigned territory is required

  • Build and maintain relationships with hospitals, case managers, physicians, and healthcare providers

  • Coordinate referrals and facilitate smooth transitions of care

  • Represent our skilled nursing and rehabilitation centers with professionalism and integrity

  • Work closely with facility leadership to support census growth and community outreach

Qualifications:

  • Experience in healthcare marketing, admissions, case management, nursing, or hospital liaison services preferred

  • Excellent communication and relationship-building skills

  • Self-motivated with the ability to manage a travel-based territory

  • Valid driver’s license and reliable transportation required
