Job Description

Own hospital referrals and regional growth through hospital discharge planners, case managers, and physicians, while aligning community engagement and admissions activity with clinical teams.

  • Hospital & Referral Outreach: Build and maintain relationships to generate referrals.
  • Hospital & Referral Outreach: Conduct regular hospital visits and coordinate smooth patient transitions.
  • Hospital & Referral Outreach: Identify new opportunities and track referral trends in the region.
  • Community Engagement & Marketing: Represent the facility at local events and health fairs.
  • Community Engagement & Marketing: Plan and execute community-based initiatives and sponsorships.
  • Community Engagement & Marketing: Develop promotional materials aligned with branding and compliance.
  • Admissions Support: Partner with admissions and clinical teams to evaluate referrals and coordinate pre-admission steps.
  • Admissions Support: Track and report referral data and admission outcomes to guide strategy.
  • Brand & Relationship Management: Serve as primary liaison with external partners and maintain knowledge of services and rehab programs.
  • Brand & Relationship Management: Monitor competitors and market trends to identify growth opportunities.
  • Minimum 2-3 years in healthcare marketing, business development, or admissions, preferably in skilled nursing, rehab, or hospital settings.
  • Nursing home experience required.
  • Strong relationships with hospital discharge planners and healthcare professionals in the Long Island region preferred.
  • Proficiency in basic communication programs; Census Pro knowledge a plus.
  • Excellent communication, interpersonal, and presentation skills.
  • Ability to work independently, prioritize effectively, and meet goals.
  • Valid driver’s license and reliable transportation; regional travel required.
  • Established relationships within hospitals, physicians’ networks, and community organizations
