Job Description
Responsibilities
- Develop and maintain referral relationships with:
- Physicians, hospitals, and discharge planners
- Skilled nursing and assisted living facilities
- Community organizations and case managers
- Educate referral sources on hospice eligibility and services
- Conduct in-person visits, presentations, and follow-ups
- Track and manage referral pipeline and activity
- Collaborate with clinical team to ensure smooth patient transitions
- Represent Haven at networking events and community functions
- Meet or exceed monthly census and referral goals
Requirements
- Experience in healthcare marketing, hospice, or home health
Preferred
- Experience in healthcare marketing, hospice, or home health preferred
- Knowledge of hospice criteria and Medicare guidelines (preferred)
