Job Summary
Own the referral network and community education to connect patients to hospice care and smooth transitions from facility to home, partnering with clinical teams to ensure compassionate, clear guidance for families.
- Referral Development: Build and maintain relationships with hospitals, facilities, physicians, and community partners.
- Family Navigation & Education: Guide families through admission with empathy, explain services, obtain consents, and set expectations.
- Community Outreach: Lead in-services and events to raise awareness and support growth in the local community.
Job Description
Responsibilities
- Build and maintain strong referral relationships with hospitals, facilities, physicians, and community partners.
- Serve as a trusted resource for hospice education and support.
- Guide patients and families through the referral and admission process.
- Meet with families to explain hospice services with empathy and clarity.
- Obtain consents for care respectfully and compassionately.
- Lead educational in-services and community outreach events.
- Help families understand what to expect from hospice care.
- Partner closely with our clinical team to ensure smooth transitions from facility to home.
- Support the continued growth of Concho Hearts Hospice in the community.
Requirements
- Minimum of 2+ years of hospice marketing experience (required)
- Proven ability to build and maintain referral relationships
- Strong communication and presentation skills
- Self-motivated, dependable, and organized
- A compassionate heart for serving patients and families
