Responsibilities
- Referral Source & Relationship Management: Build and maintain relationships with hospitals, physicians, nurse practitioners, and home care agencies; conduct regular visits to referring sources.
- Admissions Assessment & Eligibility: Assess referred patients against facility admission criteria; perform in-person assessments as needed; verify insurance.
- Discharge Coordination: Ensure a smooth hospital-to-SNF discharge process by coordinating with discharge planners, case managers, social workers, and caregivers.
- Admissions Governance: Approve admissions based on facility criteria and bed availability.
- Market Growth & Partnerships: Maintain market knowledge, identify new referral opportunities, and support the organization’s marketing plan.
- Documentation & Reporting: Keep accurate records and produce weekly activity and outcomes reports.
Requirements
- 2 years of experience in the Admissions department in a long-term care setting preferred
- Excellent organizational and communication skills
