Job Description

Responsibilities

Build and maintain relationships with Hospitals, Rehab Centers, Adult Living Facilities, Skilled Nursing Facilities, OB/GYN offices, Maternal Health Providers, Social Workers, Case Managers, and community organizations
Conduct daily in-person visits and networking activities
Identify and develop new referral sources
Respond to and follow up on service inquiries
Help guide families through service options
Participate in or support in-home assessments
Convert referrals into active clients
Expand our brand in Volusia/Flagler counties
Conduct presentations, lunch & learns, and community outreach
Attend and coordinate small events
Maintain consistent follow-up with referral partners
Track contacts, referrals, and outcomes
Maintain CRM or tracking tools
Meet weekly activity and performance goals

Requirements

Experience in healthcare marketing, home care, hospice, or related field (preferred)
Strong networking and relationship-building experience
Self-starter with the ability to drive business growth
Ability to pass a Level 2 background check
Valid driver’s license and reliable transportation
Comfortable with local travel (daily fieldwork)
Basic tech proficiency (Microsoft 365, CRM, phone communication)
