Responds via the hospice on-call system to patients in a timely manner. 2. Prioritize emergency phone calls responding to the most needy patients first. 3. Obtains report and gives report from/to Home Care and Nursing Home Program staff each day. 4. Documents appropriately all telephone calls, patient visits, telephone orders and logs, and submits in a timely manner. 5. Collaborates with the patient, family and other members of the interdisciplinary team, obtaining physician orders as is appropriate. 6. Collaborates with other health care providers such as HHA, DME pharmacy etc. to provide continuity of care. 7. Considers factors related to safety, effectiveness and cost in planning and delivering patient and family care. 8. Acquires and maintains current knowledge in Hospice Nursing Practice. 9. Collects patient and family data, documenting both initial and on-going assessments including physical, emotional, spiritual and social issues. 10. Develops a nursing plan of care that prescribes intervention to attain expected outcomes. 11. Implements the interventions identified in the plan of care. 12. Demonstrates ability to adjust to changing and crisis situations. 13. Serves as a resource to new orientees and students. 14. Shows a willingness to assume and complete additional tasks without prompting, such as admitting patients on the program as needed. 15. Ability to assess need for and make on-site visits as patient needs arise. 16. Uses good judgment in decision making consistent with the Hospice philosophy and the patient’s plan of care. EDUCATION: Baccalaureate degree preferred or sufficient credits towards same and/or equivalent experience. Participates in ongoing continuing education relevant to the area of responsibility. LICENSE:: Current registration and license to practice as a Registered Professional Nurse in New York State. WORK EXPERIENCE: Minimum two years of staff nursing. Hospice, Community Health Nursing and/or Oncology Nursing experience preferred. WORKING CONDITIONS: Works in multiple home care settings, generally but not always in well-lighted and ventilated rooms with direct patient contact. Possibly of cuts or minor burns from instruments and equipment. Some direct exposure to contagion. Possibility of strains due to moving patients or injuries from irrational patients. Risk is minimal when proper precautions are used. Clean NYS drivers license required. MENTAL DEMANDS: Applies principals of logical or scientific thinking to define problems, collect data, establish facts and draw valid conclusions in order to provide effective patient care. EQUIPMENT OPERATED: 1. Blood Pressure Machine 2. Continous Tub/Feeding Pump 3. Hoyer Lift 4. Hypothermia Blanket/Machine 5. Intravenous Infusion Pump 6. Oxygen Gauges, Tanks, Concentrator 7. Suction Equipment The most commonly used equipment is listed above. As new technology provides advances in medical care equipment list may be updated. SUPERVISION RECEIVED: General SUPERVISION EXERCISED: Responsible for the direct supervision of contracted Home Health Aides and therapist services OTHER: (Please include if appropriate, analytical ability, interpersonal skills, licenses, certifications, etc.) Works cooperatively with other departments of Staten Island University Hospital, contract and community agencies in problem solving, planning and carrying out stated patient and Hospice goals. Maintains open channels of communication and works cooperatively with other members of the health care team to share pertinent information regarding patients, to assure continuity and quality patient care. Actively participates in professional organizations and committees to keep current with changing trends in nursing and employee relations. Work on Saturdays, Sundays, Holidays or such hours as required to assure proper functioning of the Hospice. Reports for duty when called for internal or external disasters.
