Responsibilities

• Manage your own caseload and work closely with customers, carers, and partner agencies to deliver person-centred care.
• Collaborate with Occupational Therapists, Social Workers, and multi-professional teams to promote independence, choice, and wellbeing using universal services and community resources.
• Demonstrate strong organisational ability to manage caseloads in a fast-paced team.
• Communicate effectively to work collaboratively with colleagues and partners.
• Provide outcome-focused assessments to identify the right support for positive life changes and create and review support plans that make a real difference.

Requirements

• NVQ Level 4 in Health and Social Care or equivalent or the ability to work towards this qualification
• Sound understanding of the Care Act 2014
• Experience of working within a Health or Social Care setting
