Job Summary

Own and grow hospice referral networks by educating stakeholders, coordinating events, and guiding patients and families through admission.

Build relationships with physicians, hospitals, and community partners
Lead marketing events and educational sessions
Guide referral and admission processes and goals of care conversations

Job Description

Responsibilities

Build and maintain relationships with physicians, hospitals, skilled nursing facilities, discharge planners, and community partners
Plan and coordinate marketing events, educational sessions, and community luncheons
Educate healthcare stakeholders on hospice and palliative care services
Support patients, families, and referral partners throughout the referral and admission process
Provide hospice informational / goals of care conversations with patients and families
Facilitate initial patient consent process
Attend weekly meetings to report successes, share competitor insights, and identify growth opportunities
Meet or exceed performance expectations set by the Director of Business Development and Executive Director leadership

Requirements

LVN, RN, or MSW required
1–2 years of healthcare marketing/sales experience preferred
Minimum of 2 years hospice experience highly preferred
Strong familiarity with local referral partners including: Hospitals; Physicians; Board & Care (B&C); Assisted Living Facilities (ALF); Residential Care Facilities for the Elderly (RCFE); Skilled Nursing Facilities (SNF)
Excellent written and verbal communication skills
Self-directed with the ability to work independently
Determined, collaborative, and strong work ethic
Consistent, persistent, patient, and compassionate interpersonal approach
Valid driver’s license, clean driving record, and current auto insurance required
