Job Summary

Own the referral pipeline for hospice services by building relationships with physicians and facilities, coordinating education and community events, and guiding patients and families through admission and goals of care conversations.

Job Description

Responsibilities

Build relationships with physicians, hospitals, skilled nursing facilities, discharge planners, and community partners
Plan and coordinate marketing events, educational sessions, and community luncheons
Educate healthcare stakeholders on hospice and palliative care services
Support patients, families, and referral partners throughout the referral and admission process
Provide hospice informational / goals of care conversations with patients and families
Facilitate initial patient consent process
Attend weekly meetings to report successes, share competitor insights, and identify growth opportunities
Meet or exceed performance expectations set by the Director of Business Development and Executive Director leadership
Partner with Clinical team to coordinate schedule in accordance with patient Plan of Care and Medicare Guidelines
Process workflow in EMR system

Requirements

Determined, collaborative, strong work ethic.
Must have 1-2 years of experience in marketing/sales in healthcare field.
Two years’ experience in hospice preferred.
Familiarity with referral partners in the area including physicians, B&C, ALF, RCFE & SNF
Must have great communication skills and ability to work independently.
Consistent. Persistent, patient and kind with great interpersonal skills
Must have a valid driver’s license, good driving record, and current auto insurance.
Our ideal candidate will be a LVN (CA Licensed Vocation Nurse) or RN (Registered Nurse), however other candidates with solid hospice experience may be considered for this position.
LVN, RN, or MSW required
1–2 years of healthcare marketing/sales experience preferred
